Date of admission : 27/07/2022 CASE A 46 year old male patient, shepherd by occupation, resident of nalgonda came with chief complaints of  1. Shortness of breath since 6years 2.Cough since 4years which is on and off 3. Pedal oedema and facial puffiness since 3years. 4. Abdominal distension since 3years.  5. Hard stools since 10 days.  HISTORY OF PRESENT ILLNESS Patient was apparently asymptomatic 6years ago. Then he developed shortness of breath which was initially grade 1 and gradually progressed to grade 2 . Cough since 4 years which is productive with yellow colored sputum. CASE A 60 year old female , daily labourer by occupation, resident of juhugudem came to OPD with chief complaints of  C hief complaints 1.Yellowish discoloration of eyes since 25 days.  2. Generalised weakness since 25 days.  3. Fever since 25 days. HISTORY OF PRESENT ILLNESS Patient was apparently asymptomatic 20 days back and developed a fever associated with burning micturition and yellowish discoloration of eyes for which she came to our hospital and advised admission but attenders doesn't want to get admitted.  Then she used some herbal medication on every monday for 2 weeks(I.e.. Abdominal pain since 4days. 2. Shortness of breath since 3days.  HISTORY OF PRESENT ILLNESS Patient was apparently asymptomatic 10years ago , then he developed gastric problem whenever he takes alcohol for which he is using omeprazole and rabiprazole . He vomits wantedly the next day morning he took alcohol every time. 4days back he developed upper abdominal pain which was gradual in onset and progressive with abdominal distension .   HISTORY OF PAST ILLNESS Known case of Hypertension since 4years  Not a known case of DM, asthma, TB, Epilepsy.
